In this role, you will be responsible for coordinating home health aide schedules and connecting with patients to match their care needs. Duties Coordinate and manage schedules for home health aides to ensure timely and effective patient care. Communicate with patients to understand their needs and preferences, ensuring proper matching with available staff. Serve as a liaison between patients, aides, and healthcare providers to facilitate smooth scheduling and care delivery. Maintain accurate records of schedules, patient information, and staffing assignments. Address any scheduling conflicts or changes promptly to ensure continuous patient support. Provide support and guidance to staff members regarding scheduling procedures and patient requirements. Ensure compliance with all relevant policies and regulations related to home health services. Skills Bilingual proficiency is required to effectively communicate with diverse patients and staff. Strong organizational skills with the ability to manage multiple schedules efficiently. Excellent interpersonal and communication skills for connecting with patients, staff, and healthcare providers. Ability to problem-solve quickly and adapt to changing circumstances. Compassionate attitude towards patient care and staff coordination. Attention to detail in maintaining accurate records and documentation. Familiarity with healthcare or home health services is a plus but not mandatory. This role is essential in providing high-quality home health services by ensuring proper staffing aligned with patient needs.
