In this role, you will provide expert nutritional guidance, develop personalized care plans, and collaborate closely with healthcare professionals to support individuals across various settings. Duties Conduct comprehensive patient assessments to evaluate nutritional needs and develop tailored care plans. Provide evidence-based nutrition counseling and education to clients with diverse dietary requirements, including weight management, geriatrics, and chronic disease conditions. Utilize medical nutrition therapy (MNT) techniques such as tube feeding management and diagnostic evaluation to support patients’ nutritional health. Collaborate effectively with healthcare professionals—such as physicians, nurses, and therapists—to coordinate care strategies and optimize treatment plans. Document all client interactions, assessments, and care plans accurately within Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems. Implement nutrition support interventions including dietary modifications and specialized feeding protocols like tube feeding. Apply motivational interviewing techniques to encourage positive behavioral changes and enhance client engagement in their health journey. Skills Proven experience in clinical nutrition, including acute care, long-term care, hospital settings, or dietary department roles. Strong knowledge of physiology related to digestion, metabolism, and chronic disease processes. Expertise in medical nutrition therapy (MNT), weight loss programs, and managing specific dietary needs such as tube feeding or dietary restrictions. Excellent client communication skills with the ability to motivate and educate diverse populations. Skilled in using EMR/EHR systems for documentation, tracking progress, and managing electronic health records efficiently. Demonstrated supervising experience or leadership in dietary or clinical settings. Proficiency in health coaching techniques like motivational interviewing to foster sustainable lifestyle changes.
