Practice Success Manager

HBOXУдалённо · Remote

Help physician practices deliver better care, and prove it with data.

We're looking for a Practice Success Manager who is equally at home in a practice manager's office, across the table from a physician, and inside an EMR. You'll be the person practices trust to tell them where they stand on quality and what to do next.

About HBox

HBox is a fast-growing healthcare technology company. We partner with cardiology and other specialty practices across the U.S. to run remote patient monitoring and chronic care programs that keep patients healthier between visits. Our care teams, clinical experts and analytics platform work as an extension of each practice.

Backed by a growth-focused private equity partner, we're expanding from care programs into value-based care, helping practices succeed as they are paid more for outcomes and less for volume.

Why this role matters

Physician practices are increasingly measured, and paid, on the quality and cost of the care they deliver. Most know this matters, but few have the time, data or outside perspective to see exactly where they stand.

That's where you come in. You'll help practices understand their performance, find the gaps hiding in their workflows and data, and put a practical plan in place. This is a new and growing part of HBox, so you'll also help shape how we do it as we scale.

What you'll do

- Be the trusted partner. Own the relationship with each assigned practice, from the first on-site meeting onward. Build real working relationships with practice managers, physicians and care staff. - Assess where practices stand. Lead structured practice assessments: interview the team, observe clinical workflows, and review how quality data is captured and reported. - Find the story in the data. Work hands-on in practice EMRs to pull reports, check measure performance, and spot missing or inconsistent data. Partner with our analytics team on the deeper analysis. - Turn insight into action. Present clear findings and a practical improvement plan to physicians and practice leaders, in plain language and backed by their own numbers. - Drive results. Keep improvement plans moving, remove roadblocks, and run regular performance reviews with each practice. - Build the playbook. Bring what you learn in the field back to our clinical, product and analytics teams, and help shape how HBox scales this work.

What you bring

- 3+ years working directly with physician practices in healthcare quality, practice transformation, EMR implementation or training, clinical data abstraction, provider engagement, or a similar role. - A solid grasp of healthcare quality measures (for example MIPS, HEDIS, eCQMs or ACO quality reporting) and how they're calculated from chart data. - Hands-on EMR skills: running reports, knowing where data lives in the chart, and exporting data from systems such as eClinicalWorks, athenahealth, NextGen or Epic. - The confidence to build rapport with physicians and practice leaders, run a meeting, and present findings clearly. - Strong Excel skills and a habit of checking that the numbers make sense. - Organized and self-directed, comfortable managing several practices at once from a remote base. - Bachelor's degree in health administration, public health, nursing, health informatics or a related field, or equivalent experience.

Bonus points

- Cardiology experience: a cardiology practice, cath lab, cardiac registry or cardiac quality team. - A clinical background such as RN, MA or pharmacy technician. - Credentials such as CPHQ, an EMR certification, or a coding certification (CPC, CRC). - Experience with a physician organization, ACO, quality improvement organization or quality registry. - Familiarity with remote patient monitoring, chronic care management or value-based care.

Why join HBox

- Real impact, close to the patient. Your work helps practices keep patients with chronic conditions healthier and out of the hospital. - Ground floor of something new. You'll be one of the first in this role and help define it, not just follow a script. - Room to grow. As the program scales, this role can grow into leading the Practice Success team. - Backed for growth. Private equity backing gives us the resources to invest in new programs and people. - Real support behind you. Our clinical, care and analytics teams do the heavy lifting on data so you can focus on the practice. - Flexibility. Remote base with purposeful in-person time at practices: a few days a month on site, not on the road every week.
