Summary: The Waiver Case Manager supports individuals with developmental disabilities by connecting them to Indiana's state-funded waiver services. This role involves conducting assessments, developing individualized service plans, coordinating multidisciplinary care, and ensuring compliance with Medicaid and state regulations. The Case Manager acts as a client advocate, promoting independence and well-being while maintaining accurate documentation and contributing to quality improvement initiatives. Responsibilities: - Assess clients’ needs, strengths, and goals to determine eligibility. - Develop and regularly review Individualized Service Plans (ISPs). - Coordinate multidisciplinary care and maintain communication with stakeholders. - Ensure all documentation is accurate and compliant. - Monitor progress and adapt plans as needed. - Uphold Medicaid waiver program requirements. - Advocate for clients within healthcare and community systems. - Participate in training and quality improvement initiatives. - Collaborate with providers to deliver holistic support.
