If you’re a hospital liaison, community liaison, or healthcare business development professional with established relationships with discharge planners, case managers, social workers, and physician offices , this role lets you turn those relationships into steady referral growth. What You’ll Do - Build and maintain referral relationships with hospital case managers, discharge planners, social workers, hospitalists, and care coordinators - Develop and grow referrals from physician practices, including primary care, cardiology, pulmonology, orthopedics, geriatrics, and wound care - Visit assisted living and independent living communities to educate wellness and care directors on in-home care options - Educate referral partners on our home health services, admission criteria, and fast-start-of-care process - Identify new referral sources and build a qualified pipeline in your territory - Represent Ehsas at networking events, healthcare meetings, and community outreach events - Meet monthly and quarterly referral growth goals What You Bring (Required) - 3+ years in hospital liaison, community liaison, physician relations, or healthcare business development - Existing referral relationships with hospitals, physician practices, or senior living communities in Cook, DuPage, Will, or Lake County - Strong understanding of discharge planning, care transitions, and hospital referral workflows - Proven ability to grow referrals and manage a territory independently - Excellent communication, presentation, and relationship-building skills - Valid Illinois driver’s license and reliable transportation Nice to Have - Home health or hospice marketing experience - Familiarity with Medicare and Medicare Advantage Compensation and Benefits - Full-time position. - Benefits: PTO, mileage reimbursement, bonus structure, or other benefits offered. - Schedule: Monday to Friday 9am to 6pm CST Pay: $62,200.00 - $82,000.00 per year Work Location: Hybrid remote in Downers Grove, IL 60515
