This is a hands-on role suited to an experienced RN with a strong background across Hospital in the Home (HITH), NDIS and aged care/community nursing . The successful candidate will have strong clinical nursing skills and understand clinical governance, participant safety, quality service delivery and effective workforce coordination. The role combines clinical operations and coordination with the ability to provide hands-on HITH and community nursing care when required . The successful candidate will work closely with the Directors, nursing team, support workers, participants, families, hospitals and other external stakeholders to ensure services are safe, responsive, compliant and delivered to a high standard. Key responsibilities Oversee the day-to-day clinical operations of NDIS, HITH and aged/community care services Coordinate and provide clinical oversight of our HITH program Undertake HITH and community nursing visits when required Provide clinical nursing care including IV antibiotic administration, PICC and central line care, Port access and management, Surefuser/elastomeric infusion management and chemotherapy disconnects , where competent and within scope Provide complex wound assessment and management, including NPWT/VAC therapy Undertake catheter and continence care, including IDC and SPC changes Provide PEG, enteral feeding and stoma care and clinical oversight Perform comprehensive nursing assessments, observations and recognise and escalate clinical deterioration Coordinate onboarding and commencement of new participants and clients Review referrals and identify clinical, staffing, risk and service requirements Develop and review individualised Care Plans, Support Plans, risk assessments and clinical documentation Provide clinical oversight for participants with complex and high-intensity support needs Coordinate services between RNs, ENs, support workers and other members of the care team Monitor changes in participant needs and ensure timely clinical escalation Support hospital discharge and transition into community-based care Provide clinical guidance, education and competency support to nursing and support staff Liaise with hospitals, HITH teams, GPs, allied health professionals, Support Coordinators, families and other providers Maintain strong knowledge of the NDIS Practice Standards, NDIS Code of Conduct and relevant aged care requirements Support clinical governance, incident review, risk management, quality improvement and audit readiness About you Current unrestricted registration as a Registered Nurse with AHPRA Relevant nursing qualification Strong HITH nursing experience and demonstrated acute/community clinical skills Experience administering IV medications and managing PICCs, central venous access devices and infusion devices Strong wound assessment and wound management skills Competency in catheter care and other community nursing procedures Strong NDIS experience Experience within aged care, community nursing, HITH or home-based healthcare Ability to independently assess patients in the community, recognise deterioration and escalate appropriately Availability to work a minimum of four days per week Current unrestricted Australian driver's licence Current NDIS Worker Screening Check and relevant employment screening requirements Strong understanding of clinical governance, risk management, incident management and participant safeguarding Excellent clinical documentation, communication, leadership and organisational skills Ability to work autonomously while providing clinical leadership to a multidisciplinary community-based team Pay: $115,000.00 – $130,000.00 per year Work Location: In person
